Opinion | The Debate Over Transgender Care and Detransitioning

Ms. Paul cites stories of detransitioners as if they are damning to the practice of gender-affirming care as a whole. Not all detransitioners regret their transition, and not all transgender people will medically transition. By writing this article, Ms. Paul further stigmatizes health care for transgender people. What we do know is that transgender youth are under attack across the nation. Texas’s attorney general, Ken Paxton, last month demanded records from providers outside his state to single out hospitals or clinics that have treated transgender youth from Texas.

Missouri officials struck back at one of the clinics that unsuccessfully challenged new state restrictions on gender affirming care, accusing the clinic in a lawsuit of failing to provide proper care for transgender minors even before the new law took effect. Every major medical organization, including the American Medical Association, has opposed bans on gender-affirming care for minors and supported their access to medical care when treatments are administered appropriately. Doctors who treat transgender patients say those decades of use are proof the treatments are not experimental. Critics of providing gender-affirming care to minors have raised concerns about children changing their minds. Yet the evidence suggests detransitioning is not as common as opponents of transgender medical treatment for youth contend, though few studies exist and they have their weaknesses.

Why detransitioners are crucial to the science of gender care

A Canadian health provider said it couldn’t participate, citing recent threats to hospitals offering youth gender care. But as Reuters found, hard evidence on long-term outcomes for the rising numbers of people who received gender treatment as minors is very weak. Dr Marianne van der Loos, the Dutch study’s lead author, is a physician at Amsterdam University Medical Center’s Center for Expertise on Gender Dysphoria, a pioneer in gender care for adolescents. For those who also received medical treatment, detransitioning typically includes halting the hormone therapy they otherwise would receive for years. One is Max Robinson, who was 16 when she sought gender care at Kaiser in 2012.
